Advanced care planning involves discussing and documenting your preferences for medical treatment and care.

This process helps:

  • Ensure your values and desires are honored.

  • Provide clear guidance to your healthcare team and loved ones.

  • Reduce stress and uncertainty for your family during challenging times.

Reflecting on Your Values and Wishes

Take time to consider what is most important to you in terms of quality of life and medical treatment.

Here are some questions to consider:

  1. What matters most to me in life, and how can I ensure those values guide my care decisions?

  2. Who are the people I want to be involved in my care, and how can I communicate my wishes to them?

  3. How do I define quality of life for myself, and what interventions would align with that definition?

  4. What kind of environment do I want to be in if I require end-of-life care, and how can I plan for that setting?

  5. How can I ensure that my loved ones understand my wishes and feel supported in honoring them?

Communicating with Loved Ones

Have open and honest conversations with your family and healthcare providers about your preferences. It may be uncomfortable but is necessary to ensure everyone understands your wishes and can support you in making informed decisions. The next step is to have “the conversation” with your loved ones. This allows you to explain your choices and listen to their questions and concerns. 

Completing Your Advanced Care Plan

When you are ready, you can complete an Advance Care Plan, a document that lets you clearly outline your healthcare preferences, usually through an advance directive or living will. Then, you will appoint a Durable Power of Attorney for Healthcare (typically a spouse or adult child) to ensure your wishes are followed.

Once you have completed your Advance Care Plan, have it signed by two witnesses or acknowledged by a Notary Public. Afterward, ensure copies are provided to your loved ones, your physician, and your healthcare provider. 

Reviewing and Updating

As circumstances change, it is important to regularly review and update your advanced care plans to reflect any changes in your health, values, or preferences. This ensures your plan remains current and relevant.
