Starting the conversation about end-of-life decisions can be tough, but it’s one of the most important discussions you’ll have. Here are a few ways to approach it:
Find The Right Moment
Find a quiet, calm time where you won’t be interrupted. This could be after a family meal, during a quiet afternoon, or on a weekend when you’re all together. It’s best to choose a moment when emotions aren’t running high and there’s no immediate crisis. You should plan for enough time for everyone to ask questions, share their thoughts, and process the conversation without feeling rushed or overwhelmed. If it feels like too much to cover everything at once, break the conversation up into smaller discussions.
Be Honest, But Gentle:
It’s okay to acknowledge how difficult this is. You might say something like, “This is about you and I want to make sure we’re doing everything the way you’d want. Let’s talk about what matters most to you. Whatever you decide, no matter how hard it is for us, we will support you.”

Getting Legal Documents in Order
To be certain your loved one’s wishes are followed, it’s important to get the right legal documents in place. Having these documents in place means you won’t have to second-guess what your loved one would want, and you can focus on being there for them. This will give you peace of mind knowing everything is set when decisions need to be made. Some important ones are:
Advanced Directive:
This outlines what treatments your loved one does or doesn’t want, such as resuscitation or the use of ventilators.
Durable Power of Attorney:
This allows your loved one to choose someone they trust to make healthcare decisions if they’re no longer able to do so.
Living Will:
A living will provides specific instructions about end-of-life care, like whether they’d prefer life-sustaining treatments or comfort care.
When you’re ready, you can complete an Advance Care Plan, a document where you outline your healthcare preferences, usually through an advance directive or living will. After choosing a Durable Power of Attorney for Healthcare to ensure your wishes are followed, make sure to sign the document in front of two witnesses or a Notary Public. Once completed, provide copies to your loved ones, your doctor, and your healthcare provider, so everyone is on the same page.
